Case Manager

Louis Nine House, Bronx New York

Basic Functions: The Case Manager at the Louis Nine House provides outreach and counseling to the residents at the Louis Nine House supportive housing facility. This individual is part of the agency's multi-disciplinary to approach to service provision.

Responsibilities Include:

  • Conducting a comprehensive In-Take assessment for all new residents and reviewing the agency's policies and procedures with them

  • Developing treatment plans for all new tenants within 30 days of their arrival

  • Maintaining concise and accurate Client Files while preserving their confidentiality

  • Monitoring residents' medication and behavior

  • Providing tenants with necessary referrals

  • Informing tenants about on-site, agency and community resources that are available to them

  • Coordinating and running special events for the tenants

  • Maintaining a daily log of client activities

  • Participating in case conferences, staff meetings and training sessions

  • Representing the agency at assigned meetings and conferences

  • Creating monthly statistical reports

  • Working cooperatively with staff at all levels to ensure quality service provision and the development and improvement of team performance

  • Performing other duties as assigned


  • A Bachelor's Degree in social work, psychology, or other acceptable equivalent

  • Experience working with a SMI/SED population, preferably in a setting with young adults with substance abuse

  • Experience with HIV education

  • Strong understanding of the developmental needs of young adults/at-risk youth

  • Sensitivity to ethnic and cultural diversity

  • Commitment to working from a strength-based, youth-development perspective

  • Excellent written and verbal communication skills

  • Strong computer skills

  • Bi-lingual in Spanish preferred
