Senior Case Manager

BASIC FUNCTION:  The Senior Case Manager at the NCS Residence supports the Residence Director in the overall management of the program. S/he supports and advances program goals by building a strong clinical team that utilizes a multi-disciplinary approach to service provision. In addition to carrying a caseload, s/he ensures that each resident works with a Case Manager to create a Service Plan and monitors goals and outcomes.


  • Supervising and evaluating two Case Managers (task and clinical) through monthly audits of client charts, service plans, and progress notes 

  • Reporting Case Manager evaluations to the Program Director and consulting with Director on proper corrective actions

  • Maintaining a caseload of approximately 15-20 residents, providing supportive counseling addressing issues of substance use, medication adherence, housing retention, etc. 

  • Conducting a comprehensive In-Take assessment for all new residents and reviewing the agency's policies and procedures with them

  • Developing treatment plans for all new tenants within 30 days of their arrival and analyzing outcomes relative to goals

  • Monitoring residents' medication intake and collaborating with onsite psychiatrist to report progress

  • Monitoring and managing utilization of AWARDS reporting system by Case Managers

  • Training new Case Managers

  • Researching best practices and participating in trainings that will improve quality of service provided to tenants

  • Amassing referral services for clients

  • Co-facilitating staff and community meetings

  • Being on-call in the absence of the Director

  • Working cooperatively with staff at all levels to ensure quality service provision and the development and improvement of team performance

  • Performing other duties as assigned by Program Director


  • Masters in Social Work or related field

  • A minimum of three years' work experience including supervisory experience

  • Very strong interpersonal and written communication skills

  • Strong computer skills

  • Demonstrated ability to perform multiple tasks effectively in a fast-paced and challenging environment 

  • Bi-lingual in Spanish preferable

  • Prior experience working with a dually diagnosed homeless or formally homeless population in a social services residential setting preferable
