Case Manager, NCS Residence

ABOUT THE PROGRAM: The NCS Residence, a long-term, supportive housing facility on the Upper East Side of Manhattan, houses 65 formerly homeless men and women, the majority of whom are mentally ill and have been chronically homeless. Staff at the NCS Residence, including social workers, case managers, and a part-time psychiatrist, provide case management and mental health counseling. In addition, NCS provides a full range of services including money management, vocational and educational programs, and recreational activities all aimed at enhancing and supporting residents’ independent living.
REPORTS TO: Program Manager, NCS Residence
BASIC FUNCTION: The Case Manager provides supportive services, outreach and counseling to the residents at the NCS Residence Upper East Side supportive housing facility. This individual is part of our multi-disciplinary approach to service provision.
Conduct a comprehensive intake and assessment for all new residents within 30 days and assessments every year thereafter.
Review agency policies and procedures with residents.
Develop treatment plans for tenants within 30 days of their arrival and every 6 months thereafter.
Maintain accurate, yet concise, Tenant Charts including current case notes and information about each tenant’s progress with their service plan with an understanding of the confidentiality issues that surround these files.
Conduct periodic room inspections to assist tenants with maintaining a healthy living environment.
Observe residents as they take their medication and monitor adherence/non-adherence.
Provide tenants with necessary referrals, including emergencies such as hospitalization, detox, etc.
Follow up on all referrals, hospitalizations, and offsite services.
Inform tenants about on-site, agency and community resources that are available to them.
Facilitate monthly group to enhance quality of life (smoking cessation, anger management, support group, etc.)
Participate in case conferences, staff meetings and training sessions and represent the agency at assigned meetings and conferences.
Conduct weekly data entry in electronic reporting systems.
Other duties assigned.
A minimum of a Bachelor’s degree (Master’s degree preferred) in a related field such as social work or human services
Two to three years of relevant experience in social services in a residential setting with people with mental illness, substance abuse, and/or dually diagnosed formerly homeless populations.
Strong interpersonal skills including empathy, resilience and dedication.
Strong time management skills.
Knowledge of Windows, Microsoft Word, and Excel. Ability to learn new software programs.
Strong English language written and verbal communication skills; Bilingual in Spanish is a plus.
