Case Manager

BASIC FUNCTION:  The Case Manager at the NCS Residence provides outreach and counseling to the residents at the NCS Residence supportive housing facility. 

Responsibilities Include: 

  • Conducting a comprehensive intake assessment for all new residents and reviewing the agency's policies and procedures with them

  • Developing treatment plans for all new tenants within 30 days of their arrival and reevaluating plans every six months

  • Maintaining concise and accurate Client Files while preserving their confidentiality

  • Monitoring residents' medication and behavior

  • Providing tenants with necessary referrals

  • Informing tenants about on-site, agency and community resources that are available to them

  • Coordinating and running special events for the tenants

  • Maintaining a daily log of client activities

  • Participating in case conferences, staff meetings and training sessions

  • Representing the agency at assigned meetings and conferences

  • Creating monthly statistical reports

  • Working cooperatively with staff at all levels to ensure quality service provision and the development and improvement of team performance

  • Performing other duties as assigned


  • Advanced degree (Bachelor’s degree required; Master’s Degree preferred) in social work, psychology, or other acceptable equivalent

  • A minimum of two years' experience in a social service setting

  • A solid clinical background

  • Very strong interpersonal and written communication skills

  • Computer literate

  • Bi-lingual in Spanish preferable

  • Prior experience working with a homeless population and/or with people with mental illness or substance abuse preferable
